SENIOR CASE MANAGER (SSHP)

POSITION SUMMARY

The Senior Case Manager position, which requires a MSW although no work experience, provides case management services, supportive counseling, therapeutic services, encompassing substance use counseling, mental health counseling, HIV education and entitlement advocacy to clients of Iris House. The Specialist will also facilitate support groups. It is expected that this position will be in the Iris House Bronx office 3 days per week and the Iris House Central Harlem office 2 days per week.

ESSENTIAL POSITION FUNCTIONS

  1. Manage a caseload of 20-25 clients
  2. Provides case management services that include, but are not limited to: monthly assessments, quarterly assessments, quarterly care planning, counseling, outreach, case conferences, advocacy, referrals (on-site & off-site), home visits & discharge planning.  
  3. Provides case management to clients with co-occurring disorders such as substance use and HIV, providing clinical supervision to staff as needed, facilitating counseling up to a maximum of 5 groups per week.
  4. Responsible for care coordination with service providers for clients, including primary medical care and supportive services.
  5. Assists clients in advocating and obtaining benefits for which they may qualify for and identifies entitlements and benefits to which clients may be entitled.
  6. Completes weekly summaries of all clients in caseload and discuss outreach efforts for clients with whom no face to face contact has occurred.
  7. Participates in Quality Assurance committee activities related to chart audit and preparation.
  8. Other duties as assigned by the Scatter Site Housing Clinical Supervisor or the Director Housing.

POSITION REQUIREMENTS Qualifications and Skills:         

All applicants must possess an MSW from accredited university, and/or license with MSW/LCSW.  Preferred candidate should have at least one year of experience in providing case management services to HIV/AIDS infected substance using population.  Skills in group facilitation and counseling preferred. Ideal candidate should be able to employ a variety of clinical techniques, theories and skills to elicit sensitive information, handle crises and work effectively in an interdisciplinary team. Strong organizational skills and proficiency in Spanish preferred.
