Home and Community Based Services
Home and Community-Based Services (HCBS) provide opportunities for Medicaid beneficiaries to receive services in their own home or community rather than institutions or other isolated settings. These programs serve a variety of targeted populations groups, such as people with intellectual or developmental disabilities, physical disabilities, and/or mental illnesses.
Iris House offers the following services for clients who are HCBS eligible: Habilitation: This service is provided on a one-to-one basis. It is designed to assist participants in acquiring, retaining and improving skills such as communication, self-help, domestic responsibilities, health care, self-care, socialization, relationship development and use of community resources. It will allow participants to develop skills necessary for community living and to continue the process of recovery. Psychosocial Rehabilitation (PSR): PSR practice is guided by the basic philosophy of rehabilitation which holds that people with disabilities need opportunities to identify and choose for themselves their desired roles in the community with regard to living, learning, working and/or participating in social environments. PSR support focuses on four main areas of recovery: basic living skills development, psychosocial skills training, therapeutic socialization, and supported employment. Empowerment Services- Peer Support: This service uses trauma-informed non-clinical assistance to achieve long-term recovery from substance use disorder (SUD) and mental health issues, including face-to-face services, advocacy, companionship, increasing awareness of existing services in the community and interim home visits after hospitalization. Peers will be assisting and helping individuals with self-directed recovery tools, life skills regaining independence and daily tasks that will enhance the quality and security of life. Eligibility requirement includes: Being 21 years and older; have a diagnosed mental health or substance disorder and eligible for mainstream Medicaid managed care; not duals and not a person with developmental disabilities. Iris House offers services to anyone eligible for HCBS with a specialty in people living with HIV/AIDS.
