Housing Programs
A core belief at Iris House is that housing is a form of prevention: in order to improve your health, you need stable sources of food, income and housing. Many of our clients come to Iris House without the security of a permanent address, or from living situations that are simply hazardous to their health.
As part of our program to help people become healthier and live more independent lifestyles, we have three housing programs providing intensive case management and support programs.
Iris House clients who meet HRA/HASA eligibility requirements can get help in securing and maintaining housing in the community where they can live independently and access necessary services.
Iris House's HOPWA Program provides secure, appropriate, and permanent housing to low-income individuals and families affected by HIV through a scattered-site apartment model. This approach allows participants to live independently while receiving tailored supportive services that address both their immediate and long-term needs. The program is designed to ensure housing stability for people living with HIV (PLWH) by focusing on critical areas such as access to ongoing HIV primary care, behavioral health services, and the management of other chronic health conditions. Additionally, Iris House recognizes the complex challenges faced by PLWH and offers a comprehensive suite of services, including case management, substance use support, mental health counseling, and assistance with accessing benefits, to ensure that participants can thrive in their communities. By combining housing stability with these essential health and social services, the program not only helps individuals maintain their housing but also improves their overall quality of life, contributing to better health outcomes and long-term wellness.
Iris House’s I.R.I.S. Program is a New York City-funded rental assistance initiative designed to support single adults who are living with a serious mental illness or substance use disorder and who are either homeless or at imminent risk of homelessness. The program provides these vulnerable individuals with affordable housing options in the form of safe, stable apartments. Beyond housing, I.R.I.S. offers a range of supportive services aimed at helping participants achieve long-term stability and self-sufficiency. These services include mental health support, substance use treatment, case management, and assistance in accessing healthcare, employment opportunities, and other essential resources. By addressing both housing needs and the underlying health and social challenges faced by participants, the I.R.I.S. Program empowers individuals to make positive strides in their lives, ultimately reducing the risk of future homelessness and promoting overall well-being.

This comprehensive program is designed not only to offer secure housing but also to address the diverse needs of participants. Core components of the SSHP include individualized case management, which helps participants navigate healthcare, social services, and community resources. The program also offers maintenance support to ensure the housing environment is safe and well-kept. Additionally, SSHP includes substance abuse services aimed at helping individuals manage addiction and maintain their recovery. Through this multifaceted support, SSHP promotes long-term housing stability, improved health outcomes, and a higher quality of life for PLWH and their families.

To be eligible for the Scatter-Site Housing Program, clients must:
- Be a New York Resident
- Be a client of HRA/HASA
- Be referred to Iris House, Inc. by HASA
- Have the ability to function independently in an apartment
- Have need for supportive housing services
- Have willingness to pay 30% of his/her income for program fees*
